The fund provides assistance to children who are citizens of Ukraine from birth until they turn 18 years old and belong to the following categories:

• Children with disabilities.

• Orphans and half-orphans who have been left without parental care and are under the guardianship of close relatives (grandparents or other guardians).

• Children of internally displaced persons who have suffered from military actions or were forced to leave their homes.

• Children with chronic illnesses requiring constant care and treatment.

•  Please make sure to attach documents confirming the child's belonging to one of the above-mentioned categories.

The form will be processed by the fund’s staff within two working days. If needed, a representative of the fund will contact you for additional clarifications.

Familiarize yourself with the full list of documents needed for assistance after a positive decision is made by the Fund's management:

• A copy of the child's birth certificate;

• A copy of the Ukrainian passport of one parent or guardian;

• A copy of the identification code of one parent or guardian;

• A recent medical report with the diagnosis;

• An official statement from the parents or guardians requesting assistance, written consent for the public sharing of information about the child on the fund's platforms, and written consent for the use of personal data.
